Remove "age_at_consent_for_treatment" in Treatment table #232

This field is collected by the POCG-CA ARGO program but Peter (Working Group leader) did not think this is useful to include in ARGO, so we will remove it. This is an optional clinical field so it should not impact anything in clinical.
