Remove 2 fields in Comorbidity table related to prior malignancy

Description

This ticket is related to https://github.com/icgc-argo/argo-dictionary/issues/220 where fields related to prior malignancy were moved from the Donor to Comorbidity table. Propose to remove the cancer_type_prior_malignancy and age_at_prior_malignancy fields since they are redundant with existing fields in the Comorbidity table.

Proposal

Proposed updates to Comorbidity Table

• add new optional field prior_malignancy (Yes/No).
• can add a validation script that requires the ICD-10 code of the prior cancer in the comorbidity_type field if prior_malignancy is submitted as Yes.
• rename comorbidity field to comorbidity_type
• rename age_at_cormorbidity_diagnosis to age_at_diagnosis
• remove cancer_type_prior_malignancy since it is covered by the comorbidity_type field (which accepts the ICD-10 code)
• remove age_at_prior_malignancy since it is covered by the age_at_diagnosis field.
