Program level exception request - Treatment - treatment_intent, treatment_setting and treatment_type

treatment_type:

• Cases for which treatment information is missing and are deceased cannot be declared "Lost to follow-up" after primary diagnosis. "No treatment" is not a suitable response as the true value is "Unknown".
• Deceased patients with no treatment information: 69/221 (31%). Only solution for a valid submission is to remove information on death of patient and declare the patient loss to follow-up after primary diagnosis.

treatment_intent:

• Treatment intent is missing for most cases.

treatment_setting:

• Treatment setting (when applicable) is missing for most cases.

Tested in QA for all three fields with exceptions of Unknown or Not applicable. Worked as expected.
