Miscellaneous ray tracing intersection failures #26851

Bug Description

During some work on MaCaw for a journal submission I hit a few corner cases. MaCaw is stochastically sampling rays so it's not unexpected to find new corner cases A slack user also found errors on their pebble mesh

Steps to Reproduce

Run MaCaw in several 2D and 3D configurations, hit those failures Run with the pebble.jou mesh (ask me for it)

Impact

Simulation failures and biases in results The impact is limited because the software reports the problem accurately

pebble mesh issues were fixed. Macaw I need to re-visit it
