HTML improvements for paged media #559

Via CSS, and given an HTML-formatter supporting this (*), the HTML could have:

1) document-controlled headers and footers, and

2) page numbers in the table of contents.

The neccessary CSS paged media properties are described in https://www.w3.org/TR/css-page-3/. rfc2629.xslt has been implementing this for quite some time (https://github.com/reschke/xml2rfc/blob/5412ff344ae0679908390ba089436d86eb932788/rfc2629.xslt#L8761).

This is similar to 558. Supporting it would require a pretty fundamental change to the requirements that went into the original request. Revisiting whether we try to use princexml is a much larger conversation.
