Update cluster info in rfc-publication-process.md #90

Moved the info about MISSREF and generations up to the "Clusters" section. Clarified REF state.

So that I'm clear - MISSREF can only occur in a cluster (i.e. there can never be a situation where a doc is waiting for a reference outside of the RFC series) and that's why this belongs in the cluster section?

If a document is waiting for a normative reference that's outside the series and it's not available yet (rare, but can happen), we put that doc into MISSREF. A cluster is determined by unavailable normative references or by stream manager/author preference.
