Update the issue templates

Hey folks!

This PR updates the issue templates to achieve the following:

• Make issues more user friendly to non-engineer/tech folks who may not be as familiar with markdown and allow them to just create fillable fields.
• Breaks the issues into three distinct categories -- bugs, documentation, and feature requests
• Make it easier for a community manager to create and triage issues.

You can see the new issues in action on my fork here.

After a merge when clicking new issue the user will see:

Clicking on a specific issue will render the new issue template which looks like (for a feature):

I'm absolutely happy to make any changes or updates. The goal of this, and future PRs, is to make things easier for a community manager to contribute and help manage and contribute the project!
