Opt-in should not apply to main document request #34

Following the discussions at EdgeConf, and in order to enable CH-based HTML manipulations (which hopefully can reduce/eliminate the use of UA sniffing), I'm taking a stab at adding text that makes it a SHOULD for UAs to send all hints on the first request, but lets them make the hints opt-in for subsequent requests.

Following an internal discussion, I've now limited the "no-need-for-opt-in" to the 'RW' hint, since there's an immediate use-case for that for server side processing.

@yoavweiss I'm inclined to close this: the language is out of date and doesn't address the privacy angle. Let's revisit this at later date in a wider context of first-request + persistent hints? First though, I think we should get some hands-on experience with the opt-in mode.
