Re-order sidebar menu items #20

These should be re-ordered to conform to workflow. eg "logout" should be
the last item.

Cati suggests:
Patients, Upload file, Messages, Users, My team, (nest, server prefs), Logout

Re-ordered items in /views/layouts/_side_links.rhtml. Also put numbers of 
messages
and unhatched inline with link.
