Fix and clean up cross-references #21

As requested per @hasel001 , this PR fixes all previously problematic cross-references.

In addition, I have converted partially encoded cross-references like these:

<<iho-s100,part=10c>>, Table 6

Into:

<<iho-s100,part=10c,table=6>>

For semantic availability.
