Raphael Changes #22

This PR contains mostly metadata additions/modifications from Raphael from circa 24Jan2023. I have cleaned up some of the references along the way, but the main intent has been to revise content per RM comments.

In later edits, I removed two annex files (per previously adopted actions), and now I'm receiving errors upon compilation of the document. Now that I have included both Svein and Raphael's edits, I will close this PR and open a new one with everything. lhh
