add 7p shell formulation #242

@rath3t Do you think this is necessary for v1.0? Perhaps 7p-shell can be included later and we can include nonlinear Reissner Mindlin shell from your paper for v1.0 ?

max wanted to work on this. we can ask him. anyway adding me shell in a basic version can be done i suppose.
