Link release note line items back to one or more issues #929

What steps will reproduce the problem?
1. Inspect revisions.txt

What is the expected output? 
  Some means of correlating specific revision details to the "issues database".
What do you see instead?
  Changes described in such a way that the entire issues database has to be searched to connect release note items to issues, with no assurance that stopping with the first (apparent) correlation constitutes a complete connection.
  A simple [n] convention with the issue number coming in a predictable place (e.g. the end of the change description) would allow for easily generating a hyperlinked version of the file. Multiple issues could most easily be handled with a 2nd through nth [n] item, but a [n,m...,k] convention would work too.

For Arduino 1.0.1, almost all of the items in revisions.txt include a link back 
to this issues list.  The Arduino 1.0 release notes were more of a summary 
because so much had changed.  I'm closing this because I'm probably not going 
to go back and revise the old release notes at this point.
