What steps will reproduce the problem?
1. Load Game from HDD.

What is the expected output? What do you see instead?
Expected Output: Game Runs
Resulted Output: Error Message

What version of the product are you using? On what operating system?
Windows XP
Updated r304 2.2

Please provide any additional information below.
Tested on the following:
ciOS 222
ciOS 223
ciOS 224
ciOS 249
All give "disc_setusb failed".

I had the same issue but some games worked and others did not.  I solved my 
issue by fixing the typo errors on the GAMEID.

Make sure the gameid's are correct. Every game must have the same GameID in the 
filename as the game actually is. They have to be in sync!
