Create some reports that generate stats for annual reports #372

Create some pages that will produce some stats that might look good in an annual report type document. Some of these might include:

Number of machines that need to be tested/number of x-ray tubes
Total number of surveys performed
Number of machines removed
New installations
Shielding plans/evaluations performed/

A report that lists approximate number of effort hours for each machine/modality and gives a total would be a useful addition

Probably easier to do this using spreadsheets. Shelving this idea for now.

New annual report pages created. /ar/{year}/annrep will provide a count of surveys by type, count of machines by modality. and count of machines by location

/ar/cexp generates a list of the two most recent survey reports for active mammography and stereotactic breast biopsy machines for the MQSA Continued Experience report. Does not include machines that were recently removed which could still be included in the report though. Need to come up with a good method for including these machines.
