adapt to trace 0.3; fix missing "parent" argument at scope creation
depopt on trace requires 0.3

@ELLIOTTCABLE this is not replacing your PR, but it's interesting to see how manual spans can simplify our lives if instrumented code already uses some ambient context (here, lwt key storage).
