Hrm, it's 4 now, but we probably won't be making a new repo for 4.x or 5.x. There's only a split into two repos to separate out people who want no components from people who want some - https://github.com/imarc/boilerplate-components/releases
I believe that the README can be updated in each release/version. Makes the most sense to me to include the version number on the readme, as its highly visible and viewable locally. When you release v. 5 you would also update the readme. Doesnt need to be in the title, but referencing it somewhere on there will be good as we deal with legacy versions still in the wild/under support.
