Add indicator for pair_patients_run>0 and include as additional covariate. Run with rho=0.6 for now:

Results stored as "pairnew". Not much changed.

Also considered restricted choice sets only to pairs with positive referrals. Not an ideal choice set construction and specialist FEs explode.
