Support .gv filename extension #5

Some time in the Graphviz 2.21.x development cycle the official extension for 
Graphviz files 
changed from .dot to .gv and the example filenames changed. generate.sh and 
index.php need to 
be updated to recognize these files. Canviz should support both .dot and .gv 
extensions.

Partial fix committed in r83: generate.sh now handles .gv or .dot files in the 
sample graph directory, but the 
xdot file is still written with the .dot extension.

The example now supports both .dot and .gv files as of r91, and generate.sh now 
preserves the filename 
extension as of r92.
