Show table of contents at top of content when there isn't enough width #381

Feature Description

The layout should show the table of contents at smaller widths, preferably at the top of the content (below the title).

Problem/Solution

Right now the table of contents aren't visible at smaller widths.

Thanks, I'm planning to add a partial and shortcode for this in the future.
