webhelp indexing not reseted #75

When I investigated (with help from Kasun) I found that the index for
b.xml was a superset of a.xml and b.xml's index. It contained all
entries for both documents AND entries for individual stems listed page
numbers from both documents.

committed in r214, the indexer dictionary was not reseted on each transformation
