fix: Move pull request template to .github folder #96

The PR template inside the .github/PULL_REQUEST_TEMPLATE folder is not automatically inserted when opening a PR through the GitHub UI. This would require a parameter in the URL to select which template to use.

As we currently only have a single template, this PR make it the default by moving it directly into the .github folder.
