This algorithm was updated to handle cases diagnosed 2015 or later using the most recent 5 year data gathered from ACS for the applicable years, which covered 2013-2017.

The data were taken from the American Factfinder, summary table S1701: https://factfinder.census.gov/faces/tableservices/jsf/pages/productview.xhtml?pid=ACS_17_5YR_S1701&prodType=table
