Changes to 2018 Head and Neck Rules #72

  1. Fix the wording of M7 question. There are two 'are's.
  2. Skip M7 and M8 even if the histology is not in the table as long as the histology is the same.
  3. Return questionable at M7 instead of M8 if histologies are not in the table and if they are not the same.
