DEX text to address security issues #223

Based on the discussion in IETF 110, and based on the discussion in the IOAM design team:

The main changes are related to selective DEX at the encapsulating nodes, rate limiting at the IOAM transit nodes, and explicit restriction about pushing IOAM onto exported packets. Another major change throughout the document: emphasized that data may be either locally collected and/or exported. Not necessarily exported.
