With references(appendix = TRUE, part = TRUE), the appendix part fails (fixed by 4c475e1). This PR also fixes the actual title of the appendices part, for HTML and other output formats.

Also:

b2b27d0: added support for debugging in pdf_report() (exposing keep_tex and clean_supporting arguments, while maintaining same defaults as before)
516bde3: set a higher tolerance in order to better protect against overfull lines.
