Issue with Form Creation on latest Lead #26

Could possibly have to do with a subdomain issue but probably not:

Whenever a new form is created the email field is not recognized and a js alert is fired.

After creating the form the page is redirect to a 404 frontend url that looks something like this:

http://beta.atwellpublishing.com/wp-admin/undefined/post.php/?post=17&action=edit&reload=true
