Develop an intake form for future cohorts that combines the Study fields with the NIH Data Intake sheet and info that is collected from the Intro call slides and also captured in the cohort spreadsheet

Items from NIH Data Intake Sheet that need to be incorporated into the LinkML model:

There is a lot of info that NIH specifically wants to provide to the contributor that wouldn't be part of a data model. I'm thinking it might be best to leave the intake form as is, and use it to populate a spreadsheet template (that would contain the data model fields)

• Need separate fields for Investigator Name/Institution/Email/Role if multiple
• Add Role - PI vs other team members/contact people
• Multicenter?
• Funding info:
  • Agency/organization
  • Grant/contract number, if federally funded
  • NIH PO Name
  • NIH PO Email
  • NIH PO Institute/Center
  • Participating NIH Institutes/Centers that funded this study
• Geographies where subjects were enrolled or data were collected
• DSMP - data types, file formats, standards, size of data
• Method for de-identifying data
• Expected number of batches/frequency of submission
  • first & last submission dates, targeted release date
• Were all of the data collected under the auspices of informed consent as part of an IRB-Approved Protocol?
• Will you be submitting any genetic/genomic data, data that have specific requirements or restrictions for access or use, or data that would otherwise be considered "sensitive" and require NIH Data Access Committee approval to access?
• Acknowledgment statement
• Expectations
• GUID language
