Fix account lifecycle workflow #740

In particular, remove-expired-accounts property doesn't work properly. If set to false, it is not taken into account (default is true). Review also the whole workflow. Then, change documentation accordingly (https://github.com/indigo-iam/iam-website/issues/57).
