FI-2749 Screen Assessment #175

Summary

This PR fixes Github Issue https://github.com/onc-healthit/onc-certification-g10-test-kit/issues/508. This JIRA ticket https://jira.hl7.org/browse/FHIR-45319 only address US Core v6.1.0. The mapping for US Core v7 is more complicated and will be addressed by a separated HL7 FHIR ticket. At this time, US Core v7-ballot Test Suite will use the same resolution as US Core v6.1.0

Changes:

Add special test Screening Assessment Guidance to US Core v6.1.0 and v7.0.0-ballot Test Suite
Add Unit tests

Testing Guidance

Run US Core v6.1.0 Test Suite Run Test 2.49 "Screening and Assessment Guidance"

Another issue is that this change is not added into CCG yet. I am thinking to change this group to optional so it does not shown in g10. We can remove the optional tag after CCG is published. Thought?
