Add the CONTRIBUTING file to the repo #67

Following on from a discussion about the CONTRIBUTING file in issue #56 , I think we should add it to the repo. This will allow future changes to this file to be made more easily.

I've attached a proposed edition of the file, which is based on the generated version, and I then added a section on "Swim". This section is useful because some documentation files, such as the C-Cookbook, are written in (.swim) format in the repo.

Yeah, I also think that the file should be in the repo. As a plus, github supports this by showing a link to this file when creating an issue or PR: https://help.github.com/articles/setting-guidelines-for-repository-contributors/
