Fixup manpage output for headerand footer #31

After pod2man creates manpage, fix the nroff text so that the header and footer make sense and don't contain Perl artifacts.

This will probably require the name and abstract meta fields.

The to_pod phase can get these from the title/name section, or they can come from meta overrides.

We should warn if they aren't available during the to_man phase.

A warning would be nice. And also a fallback would be helpful, for example when you want to generate manpages that don't follow the classic NAME, SYNOPSIS, ... layout. Or a possibility to set the footer content yourself.
