This review is from a year ago. I was planning to pay the $4.99 to buy this app if it worked when installed as a PWA, but this review makes me think it won't. Is that still the case?

Hi, unfortunately this is a limitation of Safari - extensions aren't allowed to run on PWAs.

All you can do for now is create your own shortcut on the Home screen, but this doesn't let Safari go full screen like a PWA: https://jbscript.dev/control-panel-for-twitter#safari-faq-ios-home-screen
