This is a minor re-architecture of the gem internals for greater efficiency and less resource consumption. Major changes are:

Remove Trace model and work with spans only (and directly) with quick pass-through
Remove span staging & async span tracking to reduce overhead

Notes:

There are no API changes in this PR - all pre-existing SDK and OpenTracing APIs have been preserved
