EPIC - New 'Export NIH tables' option in Box Reports form

There will be a new button next to the actual Download charts as SVG & Download PDF on the Box Reports page. It should be named Download tables for NIH (to be confirmed).

When pressing this button it will download a readme + three tables (csv format): 1) One with one row per sample, aggregating data from the sample, its batch, etc; 2) One with one row per sample, containing information about the results obtained by the awardee on their machines; 3) One with general results of the whole panel's test (sensitivity, specificity, LOD, etc.)

(final list of columns for all the 3 tables is yet to be confirmed)

Some data we don't have within CDx, especially in table 2) (name of the protocol, units of the readout, technology used, etc.). A warning message to warn the awardee before downloading the table, that some data needs to be manually fulfilled by them, needs to be shown.

It was, yet we're still waiting the final sample files to confirm. I will update definitions after that.
