Add "name" and "location" as implicit layers when creating form fields #576

Can we include name and location as 2 default layer in the Layers tab? One for each variable? This could allow for grouping it with other data elements, moving the GPS coords in the form, changing the labels. The labels would be helpful in Tanzania. They want it to be Facilities, not "Sites".

The last sentence would be a different issue, right? By the way, I think it's a bad idea to rename the "sites" to "facilities". RM is more general than a FR.
