HK: Repair AzureAS model so I can receive notifications again

Need to update model to support modifications.

Ideally, the whole model should be updated to support PIDs along with TV 2.0 which is also outdated.

In practice, fixing the Ads Model and tables in AzureAS might be enough.

• CalendarLeads table has not been refreshed since 7/27/20

Leads partitions not updated:

• Leads Taxotere Decline Reason

• Others?

• Deleted LeadsDeclineReasons table

• Added Leads[Decline Reason] column

• Fixed Leads DNQ w Decline Reason column

• Fixed Callable column

• Fixed Cases query

• Fixed LeadVendorAds (just for enabling refreshes back on, not up-to-date with latest PID changes)

Repairing as-model-process Azure Function because LeadsDeclineReasons table is not available anymore.
