Cannot create an issue

Describe the bug /github open org/repo does nothing, not even sending the command in the chat. When using the GUI to create an issue, I'm able to find my org and my private repos, but when I try to create my issue, I get this error message (see Screenshots section below).

I've granted all permissions on both sides, Slack and GitHub. Deleted multiple times the applications and sign in successfully every time. I'm able to subscribe to the repo and when I open an issue from GitHub directly, I also have a message from the GitHub app on Slack.

Same issue on my phone via the store application

Expected behavior Create the issue using provided org/repo

+1. this dropdown to click and create also does nothing. even if this is user error, it'd be nice to have documentation about how to solve it.
