BUG? 'OK!' message in red box appears on this screen

Please see attached, appears sometimes without me catching pattern. I will update when I find out more info.

One reason that caused this is me changing the Origin of item in form below to invalid value, 3 letters

SUGGESTION: Make a validation on ORIGIN field.
