URL bar on save page error is broken

To reproduce:

• https://web.archive.org/
• Enter invalid (mistype) into "Save Page Now", e.g. google.con
• In the URL entry that shows up on the error page, enter the corrected URL and hit "Latest"

The URL goes to a bad form http://form-submit.jsp?url=http%3A%2F%2Fgoogle.com&type=replay with the entered URL in the url= parameter.
