Make this compliant with Markdown workflow #2

I ran MT's instructions on how to set up a repo for the Markdown workflow. This PR means that others will be able to run make to generate the HTML, txt and XML deliverables. See https://github.com/martinthomson/i-d-template/blob/master/doc/REPO.md

The markdown had a few syntactical issues that prevent the document generations tools (i.e. kramdown) from making progress. I fixed them up just enough to get over the bar. A bit more work would be required.
