Add pre/post workshop survey questions to this repo #96

We should add the survey questions to this repo somewhere. Not sure where or how, but it would be good to keep everything related to the workshop contained in here.

I have been searching for an open + accessible spreadsheet format to replace gsheets with little to show for it. An open format for survey is probably also out there somewhere. Unfortunately my internet search capacity seems majorly handicapped lately so I can't find anything.

For now we might just drop in a .csv or .md with the questions.
