Fixed the TOC in v?.0/index.md so internal links will work in GitHub

Deleted all HTML spans around headers, and all header permalink things (¶).
Used the Markdown TOC plugin for VS Code to auto-generate the Table of Contents (levels 2 and 3 only) for GitHub.
Changed two headings containing & to contain and instead (so it's easier to guess what the auto-generated id will be).
