same include guard - OPENJPEG_H. #424

I've noticed that openjp3d.h and openjpeg.h both define some primitives (types, 
macros), and both use the same include guard - OPENJPEG_H. Which means that 
it's not possible to include them simultaneously. Is that intentional?
