This sounds very far-fetched and almost nonsensical, but apparently the token bar causes conflicts with Multilevel tokens.
Specifically, there is an issue where moving between scenes multiple times (usually forth and then back) with a teleport can cause the player's token to disappear completely from the map (and cause some odd rendering issues), forcing a refresh.
Here is the error message tossed when this error happens.
I believe there may be some kind of conflict with the token or vision renderer updates, but I'm uncertain why.
Here are some unusual things I noted about this issue:
It can be sometimes quite inconsistent, however, teleporting in different positions can eventually force it to happen.
If the setting to "disable tokenbar" is checked for the GM (and no player has access to use it), and the GM is logged in, this doesn't appear to happen.
When the bug happens, the player's tokens (and all observed tokens by the player) will no longer be rendered (as if the token vanished) and the fog of war on the map will show it as if they have no vision. However, upon refresh, the token will have appeared in the right spot.
