UPD look and behaviour of tools menu in waitlist

Whole dropdown item is now active instead of just text.
Make menu look similar to design prototype.

UPD readme with specific node and yarn version

As brew automatically upgrading node to 10.4 breaks project (one of the modules is not compatible). At least temporarily specify version of node and yarn.

FIX autofocus vitalSigns

UPD vitalSigns fields get autofocused

FIX blood pressure validation

Due to bug in handling of component by redux form validation for blood pressure was not working correctly. Fixed by always setting initial values.

UPD allow entry of vital signs by clicking on icon
