Please add form input to issue templates. Also, please add more issue templates. #822

This is also filed under https://github.com/dear-github/dear-github/issues/211, which should be referenced to for discussion here, I guess? https://github.com/dear-github/dear-github/issues/207 tries to discuss this with the dear-github org, as you appear to be an older repository. They have more commits though, but regardless, there's definitely issue overlap going on between the two of you.

@Droogans you also reported this to GitHub right? GitHub staff doesn't read issues filed here.

It would be really nice if GitHub issue templates could use forms.
