Handle lsqb and rsqb xml entities #212

A small patch because I ran into this entity in a file

The status has been set to merged but I cannot find any references to lsqb or rsqb entities in the master or entities-pr branches? If I implemented it wrongly or even misunderstood the legality of having bracket entities in xml parsing I would certainly like to know.
