Create standard column names in data dictionary #381

@skbowd , as discussed:

[x] populate in headings from original data sources (as applicable) so they match how the data is originally downloaded.
[x] create new headings for each variable that is lowercase, utilizes underscores, and has no punctuation.
[ ] populate new headings into data spreadsheets

[x] Can you review the pink text I have in column f of the data dictionary to help condense down to 15 characters or less? I'm struggling with the last few! The ones in blue text I have a comment for you to check out also.
[x] Scan through entire column to approve how I changed the column headings. Once complete, I will populate the new headings back into the speradsheets.
