Describe the crash or bug

While staying too close from a campfire when trying to cook "Cattail Sprout" from "Oh The Biomes You'll Go", I died, but no grave was found on my death spot, or on the next flat surface either above or under. There's no death message in chat, only the achievement log from using Cattail on a campfire, and the tomb generation log. The tomb generation log gives wrong coordinates, matching a previous death. The "Death Scroll" from mod "You're in Grave Danger" also gives this same obsolete set of coordinates, and give outdated info on my inventory at the time of death, probably matching the same previous death. The grave is neither at the real death spot or at the give coordinates.

Steps to reproduce the behavior

Put "Cattail Sprout" item from "Oh The Biomes You'll Go" mod in main hand.
Stay close to a campfire.
Note coordinates.
Use Cattail Sprout on said campfire.
Die.
Check chat log, and alleged grave coordinates and death inventory.

Expected behavior

The grave should be generated at death spot, or nearby if death spot is not eligible. The death message with death reason should appear in chat. The Death scroll and grave generation log should provide accurate information.

Additional context

Origin at time of death : "Insect", from "Origins++" It's apparently a known issue with mod "You're in Grave Danger", according to it's developper. But a tricky one, since reproducable steps are not known to them. So it may not be related to the modpack, but to this individual mod. (In wich case I suppose this ticket is not in the approriate issue tracker ?)
